SUBMIT MEDICAL FORM

The following must be completed by all students before arriving on campus for classes.

All information on the medical form is highly confidential.

Forms that must be completed:

1. Medical Form.
Medical Form

2. Physical Exam – signed by doctor and completed before entering first semester. Those involved in sports must have a physical before participation.

3. Copy of Immunization Record.

*Options for Immunization Records: You may fill out this form, complete the Immunization section in the Medical Form or send a copy of original shot records. A copy of original shot record is the preferred method.
